Birth Records

Birth records can be issued to the following persons: 

• Government agencies for official purposes
• Lawful representative of person or parents
• Parents of person named
• The person named if 18 or older

All others are required to have a NYS Court Order.

Death Records

Death records can be issued to:

• Child of deceased
• Government agencies for official purposes
• Parent
• Persons with a documented lawful right or claim
• Persons with a documented medical need
• Sibling
• A spouse (at the time of the person's death)

All others are required to have a New York State Court Order.

Marriage Records

Marriage Records can be issued to;

• Either spouse
• Government agencies for official purposes
• Lawful representative of either spouse
• Others with a documented judicial or other proper purpose

Certified Copies

Certified copies of vital records may be applied for in person or through the mail with appropriate proof of eligibility. Certified copies cost $10. Payment must be made with checks drawn on a local bank or a certified check, money order or bank check.
